Generally, I don't worry about #2 since it's not an error.  I have
solved problem #1 by either 1) adding a statement (e.g., info label
on the top level "VGND GND 0 0", 2) just editing the GND or VDD symbols
and changing the name, or 3) by parameterizing the text in the GND and
VDD symbols, and changing the name of the ground or power supply node
on a per-instance basis.
